How to Prepare for your appointment:

• Valid Id- Driver's License, Federal, state or local Id card, Passport, or two of the following: Birth Certificate, Social Security card, marriage certificate, high school or college diploma, etc.  for each adult applying
• Social Security Numbers and Birth dates for everyone applying
• Income Information- W2 or last two pay stubs & any unemployment letter/stub for everyone applying
• Most recent Income tax filing- If taxes were not filed, bring last 2 pay stubs
• Immigration Documents (Naturalization Certificate, etc.)
