The CalWORKs Semester Forms must be completed EVERY semester you are enrolled in the program.

1. The CalWORKs Program Application allows the office the most current information for your file. 
2. The CalWORKs Mutual Accountability Agreement must be completed every semester in order to ensure you are aware of the program's requirements.
3. The CalWORKs Photo, Video, Statement Release allows the office permission to use any photos, video, or approved statement taken from you for any marketing purposes. 
4. The CalWORKs Waiver of Confidentiality must be completed every semester in order to ensure that the office is allowed to communicate with your County Worker, other campus programs, and spouse, if applicable. 

The CalWORKs Cash Aid Agency Verification certifies that you are currently receiving TANF Benefits, or Cash Aid, and will need to be completed and returned to the office. 

If this form stated that you are no longer receiving TANF Benefits or that you are Sanctioned, the office will be unable to service you for as long as you are off aid. 

The County HSA Monthly Activity Attendance Record is to be submitted each month as an hourly break down of your County-approved Activity hours. Using your Counselor-created ITP from the beginning of the semester and your class schedule will help with the hour designations. 

Proof of grades is also needed for your Monthly Attendance submission to your County Worker. This proof can include your Student Progress Reports, course grades, or professor emails per class. 

The County HSA Monthly Transportation Log is to be submitted each month, quantifying the mileage covered in completing your County-approved Activities and Child Care. 

*The County HSA Sworn Statement is to be submitted each month with your Monthly Activity Attendance Record, Transportation Log, and proof of grades, in lieu of obtaining signatures through the office at this time of remote accessibility.
