Below is an explanation of some of the documents that we request in order to complete your financial aid file. Some of these are forms that we have on our forms page, others are documents that are provided by the applicant (e.g. W-2, 1040EZ tax forms, etc.). Feel free to contact us if this page does not clarify the document(s) you need to provide us.

Ability to Benefit Test

You indicated on your FAFSA that you have OTHER than a high school diploma. Students applying for financial aid funds must have a high school diploma or its equivalent. If you have a high school diploma, GED, or the equivalent, please bring in the ORIGINAL document to the financial aid office..

AERC Evaluation

You indicated that you received a bachelors degree from a foreign country. An evaluation of your degree(s) is required before determining federal funding eligibility. The American Education Research Corporation (AERC) provides analysis and equivalency of foreign educational credentials to that of United States educational system. They can be contacted at 626-339-4404 or http://www.aerc-eval.com/ for more information.

Permanent Resident Card

The United States Citizenship and Immigration Services (USCIS) of the Department of Homeland Security did not have enough information to confirm that you are a noncitizen (Item 14) in an immigration status associated with the requirements of eligibility for the financial assistance for which you have applied. Please bring in your ORIGINAL permanent resident card. No photocopies are accepted.

Amend Tax Returns

You or your parents' tax return must be amended before receiving federal funding.

Approval of Appeal to Extend Aid

Your appeal to extend aid application has been received by the Appeals Committee and is currently in process. Please note that the appeals process may take up to eight (8) weeks.

Birth Certificate

Please provide your ORIGINAL birth certificate. Photocopies are not allowed.

Clarify Educational Goal

You must be a regular student enrolled in an eligible course of study leading to an associates degree, certificate of achievement, or transfer to a 4-year institution as outlined in the college catalog.

Clear up Dept of Ed Problem

The Federal Student Aid (formerly known as National Student Loan Data System NSLDS) indicates that you are in DEFAULT on one or more federal student loans.  You are not eligible to receive any federal student aid until you resolve any loan default(s).You must make six consecutive payments to your defaulted loan prior to receiving federal funds. For more information on your defaulted loan, you can contact the Default Loan and Debt Collection Agency at 800-621-3115.

Complete SAP Packet

Your suspension appeal has been approved. However, before receiving federal funds, you must demonstrate knowledge of our Satisfactory Academic Progress (SAP) policy by completing and signing the SAP packet. Incomplete packets will be returned.

Copy of Student's ID or DL

Please bring in your ORIGINAL ID or Drivers License. Photocopies will not be accepted.

Corrected Social Security Card

Your current social security card does not match your name on the official records (Birth Certificate, Permanent Resident Card, Naturalization Certificate, or Passport). Please take your official record to the Social Security Administration Office to correct your Social Security Card. The address is:

DD-214 (Military Separation Form)

Please bring a copy of your DD-214 to prove that you are an independent student for financial aid purposes.

Your FAFSA indicates conflicting information on you or your parent(s) marital status.

Father's SS Card and ID

Your father's name as reported on your FAFSA does not match the name in the Social Security Administration's (SSA) records for his Social Security Number (SSN), and/or your father's name doesn't match the name reported on their 2021 tax return. Please provide ORIGINAL SS Card and ID (ie. Drivers License). Please note that unsigned Social Security Cards will NOT be accepted.

We have not received your signature page from your FAFSA on the Web application or correction, and/or the amount you reported for you or your parents' income tax is equal to or greater than the amount you reported for their adjusted gross income.  You must sign and return this SAR form before we can determine your eligibility for federal student aid.

Letter of Intent

Our records indicate that you are not currently enrolled at Oxnard College or are enrolled at Ventura or Moorpark College. If you plan to attend Oxnard College, please provide a letter stating that you intend to register and for which semester.

Mother's SS Card and ID

The Social Security Administration (SSA) did not confirm the Social Security Number you reported for your mother in Item 67, and/or your mother's date of birth as reported on your FAFSA does not match her date of birth in the SSA records for her Social Security Number (SSN).  Please provide ORIGINAL  SS Card and ID (i.e. Drivers License). Please note that unsigned Social Security Cards will NOT be accepted.

You must provide your parents' complete 2021 W-2 forms.

The Social Security Administration did not confirm that you are a U.S. citizen.  You must provide ORIGINAL documentation of your citizenship status to the financial aid office before you can receive federal student aid. Please note that photocopies are not acceptable.

Selective Service Verification

The Selective Service reported that you have not registered with them.  If you are female or were born before 1960, registration is not required, and/ or we could not send your name to Selective Service as you requested because you did not give us enough information, you are outside the age range for registration, or you did not sign your form.  If you are male and at least 18 but not yet 26, to receive aid you must do one of the following: (1) answer "Male" to Item 21 and "Register Me" to Item 22 on this SAR and also provide information for Items 1, 2, and 9, (2) complete a Selective Service registration form at your local post office, or (3) register online at www.sss.gov.  If you are a male who is age 26 or older, you must contact Selective Service at 847-688-6888 to resolve your registration status before you can receive federal student aid.  You are exempt from registering if born before 1960.

Social Security Card Verification

Statement of Support

The information provided on your Verification Worksheet does not adequately explain how, as a self-supporting student, you were able to support yourself during January 1, 2023, through December 31st,2023. Please explain in a separate sheet of paper how your living expenses were met (i.e. room and board, food, transportation, bills, etc.).

The number you reported for your number of family members in college is significantly greater than the number you reported on your FAFSA last year.  Verify number in college that are attending at least half-time between July 1, 2016 and June 30, 2017, and will be enrolled in a degree, diploma, or certificate program.
